Report of Death

Filing delayed registration of Reports of Death of Filipinos who died abroad BEFORE 2020:

• Present original documents and submit five (5) photocopies of each
  (kindly collate and arrange into sets of 5 according to the order of documents below):

  1. Report of Death Form (ROD Form Rev. 01 24 April 2018)
     • Five (5) forms should be printed in A4 paper, originally filled out and item 26 should be notarized by the notary public.
     • Entries must be TYPEWRITTEN OR PRINTED LEGIBLY

  2. Death Certificate (original copy should be submitted to the DFA)

     For death certificates not in English language, kindly submit an official English translation

  3. Certificate of Sealing of Casket
  4. Autopsy/Embalming Report
  5. Photocopies of Passport of the deceased (data pages only)

For Reports of Death of Filipinos who died abroad in 2020 onwards:

• Please email [email protected] for initial verification if the death was already registered at the Foreign Service Post having jurisdiction over where the death took place.

*Note: Upon evaluation, additional documents may be required by the evaluating desk officer from the applicant on the requirements to determine the citizenship, identity or eligibility of the deceased for registration of death under the Philippine law.

• Consular Fee: USD 25.00 to be paid in Philippine peso at the Cashier located at the 2nd floor of the DFA Office of Consular Affairs, Bradco Avenue corner Macapagal Avenue, ASEANA Business Park, Parañaque City.
• Forms are downloadable online (consular.dfa.gov.ph/) or at Consular Records Window.
• Requirements are to be submitted at the Ground floor (Door 1, Window 1) of DFA Office of Consular Affairs, Bradco Avenue corner Macapagal Avenue, ASEANA Business Park, Parañaque City
• Processing period: Approximately 2 to 4 months
