How to send feedback?

 

Fillout the client feedback form available at the counter.

Drop the accomplished form in the designated drop box located at the front desk.

How to file a complaint?

For filing of complaints, please fill out the Public Assistance and Complaints (PAC) Form and submit to the PAC Desk Officer

Complaints may also be filed via email: [email protected] and/or [email protected] .

Kindly provide the following information:

–       Name of person being complained of

–       Incident

–       Evidence
